Program rules require applicants to make a request for services by filing an FCC Form 470. The services requested on your FCC Form 470 must describe the planned service requirements requested on your FCC Form 471. The applicant stated the funding request FRN #1999010118 was a continuation of previous year FRN #1899065521 and also confirmed the establishing FCC Form 470 was FCC Form 470 #180019764. Specifically, your FCC Form 470 #180019764 posted only for Data Transmission/Internet Access for 100MB to 500MB. Your FCC Form 471 #191008474, FRN 1999010118 is requesting 1GB Data Transmission/Internet Access. Since you did not meet this program requirement, the FRN must be denied.||MR1:On your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On 01/13/2020, you provided a contract dated 3/16/2018. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to-month services. This FRN has been changed to reference contract number 309285, Contract Award Date of 03/06/2018 and Contract Expiration Date of 06/30/2021.||MR2:The establishing FCC Form 470 Application Number was changed at the request of the applicant.
