MR1: The FRN was changed from month-to-month (MTM) to contractual with a Contract Award Date of 4/12/2015 and a Contract Expiration Date of 6/30/2016 to agree with the documentation provided during the review of your FCC Form 471 application. <><><><><> DR1: FRN is denied because you failed to provide all bids associated with this funding request. FCC Rules require applicants to retain all documentation regarding the competitive bidding process for a period of five years after the last date of service received. <><><><><> DR2: FRN is denied for failing to demonstrate that price was the primary criteria in the vendor selection process. An evaluation sheet/bid evaluation matrix was not provided during the review. Without the evaluation sheet/bid evaluation matrix, USAC is unable to determine that all bids were properly considered with price as the primary factor in the selection of the vendor. <><><><><> DR3: FCC rules require applicants to carefully consider all bids and chose the most cost effective solution. Documentation demonstrates that the applicant hindered a potential bidder from providing a responsive bid. A potential bidder sought information needed to submit a bid during the 28 day bidding window. A response was provided within the bidding window in which the information sought by the potential bidder was not provided. Therefore, the applicant did not conduct a fair and open competitive bid and funding is denied. FCC rules require applicants must be ready to accept bids once the FCC Form 470 is posted on the USAC web site. The competitive bidding process conducted was not fair and open and the FRN is denied.
