MR1: On your FCC Form 471, it was indicated the products and services are being provided on a tariff basis. On 10/20/2016, you provided a contract dated 3/1/2014. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than tariff services. This FRN has been changed to reference contract number N/A, Contract Award Date of 3/1/2014 and Contract Expiration Date of 6/30/2017. <><><><><> MR2: The FRN was modified from $269.80 per month to $244.85 per month to agree with the applicant documentation. <><><><><> DR1: The vendor selection documentation provided was created after the FCC Form 471 Certification was filed. The vendor evaluation process must be completed and documented prior to the certification of the FCC Form 471.
