MR1: FRN modified in accordance with a RAL request. <><><><><> MR2: In consultation with the applicant, the service provider has been changed to Charter Communications, LLC, SPIN number 143005817. <><><><><>MR3: The amount of the funding request was changed from $494.99/month to $360.85/month to remove: the ineligible service CB TV. <><><><><> MR4: The funding request was reduced from $360.85/month to $313.18/month to remove the costs associated with Head Start students and services for the following entities: Beaver Dam Head Start, Columbus Head Start, Fox Lake Head Start, Mauston Head Start, NECEDAH Head Start, Portage I & II Head Starts, Sauk Prairie Head Start, Wonewoc Head Start. In addition, the FCC Form 471 was modified to remove the Head Start students associated with those entities. Head Start students are not eligible for funding due to: the facility provides service to children below the state's minimum recognized age for Head Start. <><><><><> MR5: This FRN is for both Internet Access and Voice Services. According to the FCC Order 14-99, all Voice Services are subject to a phase down of support beginning in Funding Year 2015. Therefore, in order to reflect the Voice Services phase down and to conduct independent reviews of these respective services, this FRN was split. The new Voice Services FRN at the reduced discount rate of 70% is 2884601.
