MR1: The establishing FCC Form 470 Application Number was changed at the request of the applicant. <><><><><> MR2: The Contract Award Date was changed from 03/11/2013 to 03/19/2014 to agree with the documentation provided during the review of the FCC Form 471. <><><><><> DR1: Documentation provided shows a decision to extend the Calnet Rates for 12 months was made on 03/19/2014 which was prior to the Allowable Contract Date and RFP due date 03/21/2014, which is 28 days after the posting of the FCC Form 470 to the USAC website. Program rules require applicants to wait at least 28 days after posting the FCC Form 470 before selecting a service provider, signing a contract, or submitting an FCC Form 471.
