Without the evaluation sheet/bid evaluation matrix, USAC is unable to determine that price was the primary factor in the selection of the vendor. Further it is not apparent that the selection of your vendor was in accordance with the criteria stated in your selective review responses, since an evaluation sheet/bid evaluation matrix was not provided. Applicants must construct an evaluation for consideration of bids received in response to the posting of the FCC Form 470 that makes price the primary factor in the selection of a vendor.
