The FRN is denied because the applicant failed to respond to the Administrators Information Request. Despite repeated attempts, you did not provide any documentation to determine if the entity met several program rules requirements, including whether a fair and open competitive bidding process was followed, whether funding needed to pay applicant portion of the charges had been secured when you filed your FCC Form 471, and whether the entities in the Block 4 of the FCC Form 471 application had authorized their participation in the filing of the applicable FCC Form(s) 471 by the consortium. Since you did not respond to our request, we could also not determine if the FRNs are for duplicative service. Program rules prohibit USAC from funding the same products or services multiple times to the same location in the same funding year. We were also unable to determine that the cited FCC Form 470 was based on the needs of the individual schools since the FCC Form 470 did not clearly define the scope of the service for each school. Additionally, the FRN references a FCC Form 470 which has not been certified by the applicant. The FCC Form 470 Certification page must be completed to certify compliance with all Program rules.
