MR1: The FRN was changed from contractual product/services to month-to-month (MTM) service to agree with the documentation provided during the review of your FCC Form 471 application. <><><><><> MR2: Based upon review of your FCC Form 471 application, we were not able to validate your requested rural classification for Ridgeview Jr High, Ridgeview-Colfax Elem, Olympia North Elem, Tri-Valley High, Tri-Valley Elem, Heyworth High, Leroy Jr High, Leroy High, Lexington Elem, Lexington High, Lexington Jr High, Olympia West Elem, Olympia High, Olympia Middle, and Tri-Valley Middle. The urban/rural classification is determined by the Metropolitan Statistical Area (MSA) data from the US Census. The county of McLean is urban according to Table Of Rural Areas By State. USAC only uses Table Of Rural Areas By State in the SLD web site at http://www.universalservice.org/sl/applicants/step04/urban-rural.aspx to determine Rural/Urban classification. The requested discount will be modified to reflect an urban classification. The discount for Ridgeview Jr High, Ridgeview-Colfax Elem, Olympia West Elem, and Tri-Valley Middle will be modified from 70% to 60%. The discount for Olympia North Elem, Leroy Jr High, Leroy High, Lexington Elem, Lexington High, Lexington Jr High, Olympia High, and Olympia Middle will be modified from 60% to 50%. The discount for Tri-Valley High, Tri-Valley Elem, and Heyworth High will be modified from 60% to 50%. <><><><><> MR3: The discount for entity Olympia West Elementary School, entity number 72144 was decreased from 60% to 50%. The discount for entity Tri-Valley Middle, entity number 163058 was decreased from 60% to 40%. The applicant failed to supply documentation to support the requested discount. As a result of this action, the shared discount was decreased from 60% to 57%.
