MR1: Per Program rules, closed entities are not eligible for funding. The following closed entities were removed from Block 4 of the Form 471 application; Wainwright Elementary School (#115990) and Mckinley Elementary School (#115887). The student counts associated with the closed entity(ies) have been removed from the application. <><><><><> MR2: Clark County Voc Skills Center (#116405), and Cowlitz County Youth Service Center (#231747) have been removed from Block 4, Worksheet 1320676, of the Form 471 application at the request of the applicant. <><><><><> MR3: The shared discount percentage was reduced to a level that could be validated by third party data. <><><><><> MR4: The Contract Award Date was changed from 11/16/2007 to 7/18/2008 to agree with the documentation provided during the review of the Form 471. <><><><><> MR5: The Contract End Date was changed from 6/30/2012 to 7/1/2012 to agree with the documentation provided during the review of the Form 471. <><><><><> MR6: The amount of the funding request was modified from $206.62 monthly to $189.66 monthly to agree with the applicant documentation.
