MR1:On your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On4/21/2021, you provided a contract dated 3/18/21. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to-month services. This FRN has been changed to reference contract number 376946, Contract Award Date of 03/18/2021 and Contract Expiration Date of 06/30/2024.||MR2:The Contract Award Date was changed from 3/18/2021 to 3/25/2021 to agree with the documentation provided during the review of the FCC Form 471.||MR3:In consultation with the applicant, this FRN was split to conduct an independent review of the respective service being provided under the FRN. The new FRN is 2199063950. The services in the new FRN are Copper Cable Modem. The product(s) and service(s) remaining in the original FRN is Fiber Ethernet.||MR4:The FRN was modified from $38.499.95 to $38,391.55 to agree with the applicant documentation.
