FCDL Comment:
MR1: The Contract Award Date was changed from 2/6/06 to 2/16/06 to agree with the applicant documentation. <><><><><> MR2: FRN modified in accordance with a RAL request. <><><><><> MR3: The FRN was modified from $354,306.52 to $411,300.11 per month and from $0 one time charge to $92,000 one time charge to agree with the applicant documentation provided in the RAL. <><><><><> MR4: The FRN was modified from $411,300.31 to $312,806.80 per month and from one time charge of $92,000 to $69,968.40 based on vendor quote and PRDE's determination that there were 1,172 eligible entities after removal of additional 13 entities that did not have a dated survey. <><><><><> MR5: The dollars requested were changed to reflect the correct number of eligible entities. The original Block 4 included 1,519 entities. The applicant provided documentation supporting the eligibility of 1,196 entities. The applicant later requested that this number be reduced to 1,185. USAC then reviewed site surveys and certifications and determined that 1,172 entities should be considered for eligibility in FY2008 after 13 entities were deemed ineligible and removed from Block 4. A list of the entities included in this FRN is being sent under separate cover.
