FCDL Comment:
MR1: The dollars requested were changed to reflect the correct number of eligible entities. The original Block 4 included 1,541 entities. The applicant provided documentation supporting the eligibility of 1,196 entities. The applicant later requested that this number be reduced to 1,185. USAC then reviewed site surveys and certifications and determined that 1,172 entities should be considered for eligibility in FY2007 after 13 entities were deemed ineligible and removed from Block 4. 659 entities have a USAC determined Ready Date prior to 08/01/2007 and are deemed eligible for FY2007. The applicant indicated that ten entities would not be receiving service on this FRN. This results in 649 eligible entities for this FRN. A list of the entities included in this FRN is being sent under separate cover. <><><><><> MR2: Based on eHealth reports provided by the applicant, the dollars requested were reduced to remove one month of inactivity for entity 206781 (ANTONIO BADILLO HERNANDEZ) and three months of inactivity for entity 207122 (BERWIND SUPERIOR). <><><><><> MR3: This FRN has been split in order to remove the funding for those entities not eligible for E-rate support as of 07/01/2007. The entities eligible for part of FY2007 have been moved to FRNs 1787183, 1787192 and 1787420 to reflect the appropriate Service Start Date with their respective eligibility dates. <><><><><> MR4: The FRN was modified from $354,309.72 to $149,141.44 to agree with the applicant documentation and removal of costs associated with the USAC determined ineligible entities.
