MR1: Based upon review of your FCC Form 471 application, we were not able to validate your requested rural classification for LA CREOLE MIDDLE SCHOOL. The urban/rural classification is determined by the Metropolitan Statistical Area (MSA) data from the US Census. The county of Polk is urban according to Table Of Rural Areas By State. USAC only uses Table Of Rural Areas By State in the SLD web site at http://www.universalservice.org/sl/applicants/step04/urban-rural.aspx to determine Rural/Urban classification. The requested discount will be modified to reflect an urban classification. The discount for LA CREOLE MIDDLE SCHOOL will be modified from 70% to 60% <><><><><>< MR2: The discount percentage for PERRYDALE ELEMENTARY SCHOOL and PERRYDALE HIGH SCHOOL was decreased from 80% to 60%, The discount percentage for OPPORTUNITY HOUSE ALT PROGRAM was decreased from 60% to 20%, The discount percentage for CASCADE HIGH SCHOOL was decreased from 90% to 60% the applicant failed to provide third party documentation to support the requested amount. As a result the shared discount has been reduced from 79% to 76%.
