The student counts associated with the closed entity(ies) have been removed from the application. <><><><><> MR2: DENVER PUBLIC SCHOOLS REMINGTON (#16067538) has been added to Block 4, Worksheet 1294878, of the Form 471 application at the request of the applicant. <><><><><> MR3: The establishing Form 470 Application Number was changed at the request of the applicant. <><><><><> MR4: The FRN was modified from $695.52 per month to $659.05 per month to agree with the applicant documentation. <><><><><> MR5: The amount of the funding request was changed from $659.05 per month to $612.04 per month to remove: the ineligible product(s) or service(s) Upgrade Fees for $3.00 per month, Equipment Fees for $35.73 per month, Restoral Fees for $5.83 per month, Late Fees for $2.45 per month. <><><><><> DR1: FCC Rules require applicants to retain all documentation regarding the competitive bidding process for the specified period of time. During application review, you were asked to provide all bids and any vendor evaluation documentation associated with this funding request and you failed to do so.
