DR1:During the Selective Review you failed to provide all bids associated with this funding request. FCC Rules require applicants to retain all documentation regarding the competitive bidding process for a period of ten years after the last date of service received. ||DR2:Applicants are required to conduct a fair and open competitive bid process. Applicants are also prohibited from including the manufacturer’s name or brand on their FCC Form 470 or in the requests for proposals (RFPs) without including the words “or equivalent” to describe the requested service or product. During the review, it was determined that the applicant included the manufacturer’s or brand name for the requested products or services without including “or equivalent.” Therefore, this FRN is denied.||MR1:The FRN was modified from $19,053.49/month to $18,949.40/month to agree with the applicant documentation.||MR2:The amount of the funding request was changed from $18,949.40/month to $18,926.50/month to remove the ineligible service(s): directory assistance for $22.90/month.
