MR1:On your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On 10/03/2017, you provided a contract dated 05/05/2017. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to-month services. This FRN has been changed to reference Contract Award Date of 5/5/2017 and Contract Expiration Date of 6/30/2018.||MR2:Based on the review of your member entity(ies), the entities Alternative School/Yes Academy (170011892) and ST MARY'S EARLY LEARNING CENTER (16082457) have closed as a school site. Per program rules, closed entities are not eligible for funding. The funding request was modified from $10,797.93/month to $10,381.80/month to remove the cost associated with the closed school site.||MR3:MADISON AVENUE KINDERGARTEN CENTER have been removed from the FRN as a recipients of service at the request of the applicant.
